What Is Vein Disease?

Vein diseases including varicose veins and chronic venous insufficiency (CVI) can lead to uncomfortable symptoms, affecting your quality of life. CVI is a disease that occurs slowly over time and is typically associated with a strong family history of vein disease.  Most commonly one parent or more will have a history of varicose veins.  Vein disease is also connected to certain lifestyle factors, including prolonged standing or sitting in stationary positions, aging, smoking, and excess weight. A history of deep vein thrombosis (DVT) can also be associated with the development of varicose veins through diseased valves. 

All of these conditions can damage the valves in your leg veins, which are responsible for keeping blood flowing in the correct direction. When they become damaged through either direct injury or wear and tear, blood can flow towards the legs with gravity and pool in your feet and legs (“leaky veins”), making it harder to pump back toward your heart. 

The cause of vein disease is important, however the consequences of untreated advanced vein disease is of greater concern.   Varicose veins and spider veins are the most obvious physical symptom, but patients should be aware of common symptoms including leg heaviness, leg cramps, charlie horses, restless legs, skin discoloration and leg swelling.

  • Venous insufficiency: Damage to your vein valves over time leads to CVI, which in turn leads to spider veins and varicose veins, along with many other symptoms.  Vein disease is characterized by stages and anyone suffering from pain, swelling or skin changes is recommended to see a vein specialist for further evaluation.   

  • Spider veins: Can be cosmetic or the visible sign of underlying venous insufficiency.  An increasing appearance of spider veins or other visible signs of vascular disease should be checked by a specialist.
  • Varicose veins: Unlike spider veins, varicose veins are bulging, twisted veins that stick protrude from the skin. On an ultrasound, most varicose veins can be easily traced to the source confirming a diagnosis of venous insufficiency.  Varicose veins may be the cause of your leg pain, swelling, or heaviness and are easily correctable once identified. 

  • Leg heaviness: Weighted, tired legs are commonly the result of venous insufficiency.  A bothersome symptom many patients become accustomed to until more significant problems arise including leg swelling or skin discoloration.  Unexplained symptoms like leg heaviness or cramps is a valid reason to have an ultrasound to assess for vein disease.  

  • Leg cramps: Leg cramps, restless legs, charlie horses are not uncommon with vein disease.  Despite mineral supplementation with magnesium, hydration, or even certain nerve acting medications, the pain persists.   Venous insufficiency is the most common cause of restless legs and is often overlooked.  
  • Leg swelling: This is a difficult one but known result of untreated vein disease.  If you have unexplained swelling or edema, a workup for vein disease including a venous reflux ultrasound is recommended.

How Do I Know I Need Vein Treatment?

The first thing is getting an expert evaluation from someone with education in vascular medicine.  This can be a vascular surgeon, cardiovascular specialist or vascular medicine specialist with certification through the American Board of Vascular and Lymphatic Medicine.  A pain management doctor is not qualified to evaluate or treat vein disease and it is not within the scope of their practice.  Patients seeking unqualified care risk unnecessary vein treatments and potentially complications such as DVT, permanent scarring and nerve injuries.

Sclerotherapy

For small spider veins (or telangiectasias), our physicians use liquid sclerotherapy. This method uses tiny needles to inject a sclerosing agent directly into the spider veins.  The medication irritates the targeted vessels, causing them to collapse. With time, the body reabsorbs the treated veins.  Most patients require two to four separate sessions of sclerotherapy for desired results. 

Radiofrequency Ablation (RFA)

Radiofrequency ablation (RFA) is the gold standard treating underlying venous insufficiency.  The usual target is a saphenous vein that demonstrates abnormal flow on an ultrasound and leads to varicose veins.  It is a medical technique intended to relieve symptoms of vein disease and improve venous circulation in the legs.  The treatment is minimally-invasive and with proper expertise can be performed directly in the office with local anesthesia only.  

VenaSeal™ 

An alternative to RFA ablation, VenaSeal is also a catheter based technique that uses a medial adhesive to close the targeted saphenous veins.  It is an FDA-approved method for treating varicose veins that does not use lasers, heat, or other chemicals. At our vein centers, VenaSeal is used in select cases only based on our experience.

Foam Sclerotherapy 

Foam sclerotherapy uses polidocanol endovenous microfoam (PEM) in higher concentrations to target large varicosities. The sclerosant is delivered in a foam consistency to treat varicose veins.  In our clinic, foam sclerotherapy is used in combination with RFA ablation for the best results.  It is also a useful medication to treat recurrent vein disease when RFA is not suitable.

Phlebectomy

Sometimes, the best way to remove large bulky varicose veins is not to inject them but to remove them. It is still a minimally-invasive procedure and far different than vein stripping.  The procedure can be done in the office or surgical center using local anesthetic.  Phlebectomy involves removing problem veins using a small hook inserted through tiny punctures in your skin. The advantage of phlebectomy is the ability to achieve rapid resolution of large varicose veins.
